St. Mary's Wellness Center Membership Rates
Effective - January 1, 2012

*Family Membership Policy: Family memberships are defined as two or more family members that reside at the same address. Family membership is a Couple Membership with one child between ages 13 – 18 added on to the membership. Additional children may be added to the membership for $5.00 per child per month.

**Application Fee: Charged to new members and members who rejoin after 6 months of inactivity.

***St. Mary's employees: Cancellations of payroll deductions require your signature on the payroll deduction authorization form. Your membership will not be cancelled until the form is signed and returned to the Wellness Center. PRN employees cannot pay via payroll deduction.

Corporate/Student#: Discounted corporate memberships are available to a business with 5 or more employees enrolled as members. Students must present valid student ID.

Automatic Draft Policy&: The automatic draft payment option is provided as a convenience to those wishing to take advantage of regular monthly payments. First month’s payment must be made via cash, check or credit upon enrollment. Monthly draft may be cancelled without penalty with written notice received by the first of the month. Those enrolled in quarterly, biannual or annual membership plans may take advantage of the convenience of regular monthly payments. If a client in a long term plan chooses to terminate the automatic draft, the remaining balance will be due immediately.

Day Pass (non-guest):  $10.00
